This document summarizes various industry trends over time on Part C and D measures using CMS data. Hover over plots for detailed information.

Throughout the text, the terms “Part C rating” and “Part D rating” are used to describe the overall Part C and Part D ratings for applicable contracts. “MAPD rating” refers to the overall plan star rating, applicable only to contracts rated on both Part C and Part D measures. While CMS awards MAPD and Parts C and D ratings in half-star increments, these analyses aggregate ratings at the lesser whole star level for simplicity. For example, both 4- and 4.5-star contracts are pooled as a combined 4-star category.


(1) Overview of Contracts

These tables provide counts of contracts receiving MAPD and Part D star ratings, and counts of contracts that were not rated due to being too new, too small, or the rating being inapplicable to the contract.

MAPD Ratings


Of the 395 contracts with an MAPD rating in both 2019 and 2018,

  • 65 earned higher overall ratings in 2019 compared to 2018,

  • 85 earned lower overall ratings in 2019 compared to 2018, and

  • 245 maintained the same rating in both years.

Part D Ratings


Of the 420 MAPD contracts with a Part D rating in both 2019 and 2018,

  • 78 earned higher Part D ratings in 2019 compared to 2018,

  • 171 earned lower Part D ratings in 2019 compared to 2018, and

  • 171 maintained the same rating in both years.


Of the 52 PDP contracts with a Part D rating in both 2019 and 2018,

  • 22 earned higher Part D ratings in 2019 compared to 2018,

  • 11 earned lower Part D ratings in 2019 compared to 2018, and

  • 19 maintained the same rating in both years.


(5) Adherence Measure Star Ratings Relative to Part D Rating

The plots below display the current year (2019) distribution of selected measure star ratings by Part D star rating. Contracts with higher adherence measure ratings tend to have higher Part D ratings.

(6) Total 4+ Star Adherence Measures by Part D Rating

All Contracts

The plot below compares the percentage of contracts with 4+ stars on up to 4 total adherence therapies (Diabetes Medications, RASA, Statins, and SUPD) by Part D star rating in the current year (2019). For example, 77.3% of contracts with a 5-star Part D rating have 4 or 5-star ratings on all four of the Diabetes Medications, RASA, Statins, and SUPD measures. An additional 9.1% of contracts with a 5-star Part D rating have 4 or 5-star ratings on three of the measures.

The plot below compares the percentage of contracts with 4+ stars on up to 4 total adherence therapies (Diabetes Medications, RASA, Statins, and SUPD) by MAPD star rating in the current year (2019). The interpretation is similar to the description given in section 6.

The heat maps below emphasize the large impact of triple weighted measures on Part D rating in the current year (2019). The x-axis indicates the star rating of the individual Part D measures on the y-axis, and the grid is colored according to mean Part D star rating. Lighter colors indicate a higher enrollment-weighted mean Part D star rating. Measure weights are indicated in parentheses on the y-axis.

(9) Part C Measure Ratings Compared to Part D Rating

The heat map below repeats the display type above, but with individual Part C measures on the y-axis. Data are displayed for the current year (2019). Again, lighter colors indicate a higher enrollment-weighted mean Part D star rating and measure weights are shown in parentheses on the y-axis. We observe that higher Part C measure ratings are associated with higher Part D ratings.

These plots display the mean change in therapy adherence rates across industry contracts from 2018 to 2019, based on 2018 therapy star ratings. The horizontal rule indicates the average industry change, weighted by the total contracts in each star category. The number of contracts in each category is shown for each bar along the x-axis.
