Member Identified

Powered by RxAnte patented analytics, health plan members are identified who are at risk for non-adherence or health complications from complex medication regimens and can receive the most benefit from Mosaic's pharmacy service.

Member Enrolled

Eligible health plan members are contacted by our outreach team to review Mosaic Pharmacy Service and  how it can benefit them and their caregivers. Interested members are then scheduled for an onboarding visit.

Onboarding Visit

We conduct an initial clinical review to determine the member’s pharmacy needs and provide data-driven recommendations on improving medication adherence. This involves: - Eliminating Drug Interactions - De-Prescribe Unnecessary Medications - Offering Less Expensive Alternatives - Referrals to Risk Adjustment, Care Management & SDOH Programs

Medication Delivery & Ongoing Clinical Reviews

Mosaic ships medications to the patient’s door every 30 days. Two weeks before the next shipment, our pharmacy team calls the patient to review any medication changes, checks for potential drug interaction or other issues and answers any questions. - Delivery in special adherence packaging - 30-day supplies, delivered to home - Pre-sorted dose pouches
