We studied the impact of a pharmacy care program for patients with conditions such as heart disease and diabetes. Program participants were more likely to refill their medications on time and had fewer hospitalizations compared with similar patients who were not offered the program. Most of the drop in hospitalizations was because the program participants were less likely to have new cardiometabolic problems.
