Filing a Non-Compliance Complaint

Reporting a Privacy/Security Event

Please provide the best contact information to locate you quickly should our incident response team need additional details.

  • Please fill out the Incident Report Form and provide as much detail as possible.
  • A copy of the report will be sent to our General Counsel & Chief Privacy Officer and Sr. Director IT Infrastructure and Information Security Officer.

Reporting Non-Compliance & Fraud

Reporters may remain anonymous if desired. RxAnte ensures non-retaliation against those who report suspected misconduct in good faith.

To make a report online:

  • Please fill out the Incident Report Form and provide as much detail as possible.
  • A copy of the report will be sent to our General Counsel & Chief Privacy Officer and Sr. Director IT Infrastructure and Information Security Officer.

To make a report via phone:

  • Call the Non-Compliance & Fraud Hotline at 1-844-233-9086 and leave a message providing as much detail as possible.
  • Your message will be sent to our General Counsel & Chief Privacy Officer and Sr. Director IT Infrastructure and Information Security Officer.
