Learn about the implementation, design, and outcomes of an innovative technology-enabled, value-based pharmacy care management program within a regional health plan. Eligible members are medically complex, socially vulnerable, and predicted to incur high avoidable costs of care. The goals of the program are to support the pharmacy care needs of this frail population, improve pharmacy quality scores, prevent unplanned care events, and lower total cost of care. Program support includes pharmacy care services provided to plan members, their caregivers, their providers, and in conjunction with health plan care navigators. The program also pioneers a new value-based payment model for pharmacy care services.
Attendees will learn how at-risk members are identified, how technology enables the pharmacy care management services provided, the value-based program design, and patient case studies from the front lines.
